Patient must have a valid prescription for VSL#3^® Unflavored Powders (450 billion CFU/packet) or VSL#3-DS^® Powders (900 billion CFU/sachet). Offer is limited to 2 boxes of 30 packets per calendar month for VSL#3^® Unflavored Powders and 3 boxes of 20 sachets per calendar month for VSL#3-DS^® Powders. No other purchase is necessary. The coupon program is not health insurance and the patient is responsible for complying with any obligations as may be required by his/her insurance provider.
